You must complete all 5 steps to receive your $30 rebate check:

  1. Get an eye exam from an Eye Care Professional AND purchase a year's supply of any ACUVUE® Brand Contact Lens from CLE Contact Lenses Inc.

  2. Complete the $30 Rebate certificate provided by CLE Contact Lenses Inc., which will be included with your shipment, and attach:
    - 4 box tops of any ACUVUE® product OR
    acuvue settlement - 2 box tops of ACUVUE® TORIC

  3. Attach receipt(s) for the eye exam AND product purchase.

  4. Mail to: ACUVUE® Brand Contact Lenses, P.O. Box 5878, Clinton, IA 52736-5878

  5. Keep a copy of your paperwork for your records.
